and any affiliated mobile applications (the “Apps”), and Podolski Psychiatry LLC and

all references to Podolski Psychiatry LLC include Podolski Psychiatry.com, Podolski Psychiatry and

PodolskiPsychiatry.com. The Podolski Psychiatry application is a tele-health patient portal application.

By using the Podolski Psychiatry application, you consent to the data practices described in this

In order to better provide you with products and services offered, Podolski Psychiatry may collect

communication. In order to improve our Services, we may receive a notification when you open an

email from Podolski Psychiatry or click on a link therein.

If you would like to stop receiving marketing or promotional communications via email from Podolski Psychiatry

and/or by updating any privacy information. Your continued use of the application and/or Services

available after such modifications will constitute your: (a) acknowledgment of the modified Privacy
